New Fields in Review Form (ESM?)

I searched the forum for an answer, but couldn't get anything.

I'd ilke to add a couple fields to my review form, namely: name, and email.
Then, when the review is submitted, on the detail page, the review appears with the individual's name with a link to there email addresses. (note: i've disabled having to register to do a review)

How / where do i add these variables? how can i go about doing this? I notice ESM asked about something like this 3 years ago without much of a response...any advice?

the review feature needs a lot of work and bug fixes...

Alright Iguess i'm on my own - I know Zubby actually custom coded something like this for me earlier - but i lost it - and wanted to see if anyone knew something like this

yep, I have given up trying to keep it clean.

my situation is too busy this week and maybe even next week. what additional fields are you looking for and what do you need them to do??
