check the owner of the file "chmod abc:abc 404.html" ... or if file exists !

May be the file is 0 length size ?

Try to put the file via FTP again in a AUTO MODE or TEXT MODE.

Seems like all details are not working.

Look at your .htaccess file - may be this is the problem ?

Also try to update all your categories(Update Category Path), keywords (Update Tags Cloud) and submitted sites (Update Number of links).

If it wont help you Dody should look at it.
